Members may schedule qualifying primary care office visits as needed during an active membership. A $30 fee applies to each visit.

MEMBERSHIP TERMS
Membership is $59.99 per month and is non-transferable. Membership benefits may not be shared, transferred, or assigned to another individual.
Members must be 18 years of age or older to enter into the membership agreement. Services provided to minors require appropriate parental or legal guardian consent.
Membership benefits are intended to support consistent, ongoing primary care and wellness. Benefits are available only while the membership remains active and in good standing.
The monthly membership fee does not include the $30 office visit fee. Each qualifying primary care office visit is $30, with no set monthly or annual limit on qualifying visits while the membership is active.
Controlled Substance Policy: To ensure compliance with federal and state regulations, appointments related to the evaluation, prescribing, refill, or management of controlled substances are not included in the OWA Care Plus Membership and are subject to standard office visit fees.

Automatic Payment Agreement
Membership fees are billed monthly in advance using the payment method on file.
Membership automatically renews each month unless canceled according to the terms of this agreement. A 30-day written notice is required to cancel membership. Cancellation requests should be submitted to:

OWA reserves the right to update membership pricing, included benefits, discounts, or membership structure with 30 days’ written notice.
Cancellation & Commitment
- 3-month minimum membership commitment required
- Early cancellation before completion of the minimum commitment will result in a fee equal to one month’s membership
- A 30-day written cancellation notice is required
- Membership benefits remain active through the final paid billing cycle
- Membership fees already paid are non-refundable except where otherwise required by law
Termination or Suspension
OWA reserves the right to suspend or terminate membership when appropriate, including when:
- Membership agreement terms are violated
- Payment is delinquent for more than 30 days
- False or materially inaccurate information is provided
- Inappropriate, threatening, abusive, or disruptive behavior occurs
- Continued participation is inconsistent with clinic policies or applicable requirements
General Terms & Conditions
Membership benefits and discounts cannot be combined with other promotions, discounts, packages, or special pricing unless specifically stated by OWA.
Membership benefits are available only during an active membership and do not accumulate or roll over unless specifically stated.
Membership services and discounts are non-transferable and non-refundable.
Missed appointments or appointments canceled outside OWA’s cancellation policy may be subject to applicable missed-appointment or late-cancellation fees.
Membership billing cycles begin on the date of enrollment, not on the first day of the calendar month.
Certain medications, laboratory testing, pathology, supplies, procedures, advanced treatments, or upgraded services may require additional fees.
The membership does not guarantee same-day appointments or availability of a particular provider.
Important Insurance Notice
The OWA Primary Care Membership is not health insurance and is not a substitute for comprehensive health insurance.
The membership does not cover hospital care, emergency department services, specialist care, imaging, outside laboratory services, prescription medications, or other services not specifically identified as membership benefits.
Cash-pay/member services generally are not submitted to insurance and may not count toward a patient’s insurance deductible or out-of-pocket maximum. Patients with health insurance should review their individual plan requirements.

By signing up, I acknowledge that I have read, understand, and agree to the terms and conditions of the OWA Primary Care Membership Agreement. I understand that the membership fee is $59.99 per month, that qualifying primary care office visits are $30 per visit with no set visit limit, and that additional services, testing, medications, procedures, and laboratory fees may result in additional charges.
I authorize OWA to charge my payment method on file according to the membership and automatic payment terms described above.
I understand that this membership is designed to provide more affordable and consistent access to routine primary care and eligible wellness services and is not health insurance.
